How to answer the questions
Green
In place, working, and you're in control. You could prove it to someone tomorrow.
Amber
Partly there. It works, as long as nobody asks how.
Red
Missing, risky, or you don't know. You'd rather nobody asked.
Be honest on the uncomfortable rows. Being honest means a better report. “I don't know” counts as Red. That isn't a failure, it just tells you where to look first. If you can't decide between two colours, pick the worse one. If a question doesn't fit your business, mark it N/A and it's left out of your counts.
